This form, once completed will allow your child to be accepted onto our waiting list. The Nursery Manager will then be in contact with you if spaces allow for changes to be made to your child's current hours.

We will need to have one calendar month’s notice if you want to make any changes to your child's hours.

It is at the manager's discretion for any changes to be made sooner than the notice period, this is likely to be under exceptional circumstances particularly where reduced hours are taking effect.
